Due to the COVID-19 Pandemic, screening guidelines have been put in place by the North Carolina Department of Health and Human Services (NCDHHS) to prevent or slow the spread of the virus in schools.  This form must be completed once by each car rider or student driver for the 2020-2021 school year.  

This attestation form serves as a way to inform parents and/or guardians of the questions that should be considered daily with your child prior to sending them to school.
